It’s almost time for summer camp! Scouts, remember to complete your summer camp prerequisites if you have any!

Here’s an update regarding all the administrative requirements for summer camp:

SCOUTS

• TWO COPIES OF Forms A, B, and C to Mrs. Sweeney by 6/22/25.
• Form C must be signed by a healthcare provider.
• Please also provide two copies of the front & back of the insurance cards and immunization records
• Additionally, for this camp, all Scouts and Adults will need to provide the supplemental medical form: Laurel Highlands Council Supplemental Medical Form (2025 Update).
• Evidence of their Merit Badge Pre-Requisite Work, including proof of completion as noted in the camp guidebook​.
• Signed Troop Activity Consent Form.

SCOUTS PARTICIPATING IN HIGH ADVENTURE

• If your Scout is participating in high adventure day treks, you will need to provide AN EXTRA COPY OF ALL MEDICAL FORMS – that means 2 copies per our usual policy plus another copy for the camp administrators. You will also need 2 copies of any appropriate waivers: Waivers for the Eagle Base Day Trek Program

ADULTS

• TWO COPIES OF Forms A, B, and C to Mrs. Sweeney by 6/22/25.
• Form C must be signed by a healthcare provider.
• Please also provide two copies of the front & back of the insurance cards and immunization records (adults only need the date of their last tetanus shot)
• Additionally, for this camp, all Scouts and Adults will need to provide the supplemental medical form: Laurel Highlands Council Supplemental Medical Form (2025 Update).
• A valid copy of your Youth Protection Training Completion Certificate
• A signed copy of the LHC Out of State Verification Form
